- Both parents will have to sign the form.
- Submit the completed form, your child's birth certificate and both parents' NRICs to your child's primary school.
- School staff will enter your choices into the DSA-Sec Portal.
- You will be given a Verification Slip (VS) endorsed with the school stamp.
- Double-check the slip and inform the staff if there are any mistakes found so that amendments can be made.
- Sign the VS and keep a copy for your records.
