Joint Admissions Exercise Request Form

Note: 2022 JAE application phase has ended.

This form is for applicants who are unable to complete the online application through the Joint Admissions Exercise Internet System (JAE-IS). It will be available from 3pm on Wednesday, 12 Jan 2022 to 4pm on Monday, 17 Jan 2022.

If you have lost your Form A

As a security measure, you should submit a request to notify us. We will deactivate your JAE PIN to prevent any unauthorised access to the JAE-IS and reprint your Form A, if required.

You will need to upload both sides of your NRIC.

Note: You can still access the JAE-IS using your Singpass.

If you are unable to log in to the JAE-IS

You can request for assistance if your JAE-IS account is locked or unable to log into JAE-IS using JAE PIN.

You will need to upload both sides of your NRIC.

If you have changed your residential address

You can request to update your residential address.

You will need to upload both sides of your NRIC.

If you need to update your CCA grade

You can request to update your CCA grade.

You will need to upload the following documents:

  • Both sides of your NRIC
  • CCA certificate or letter from school
If you need to update your citizenship

You can request to update citizenship if you have changed your citizenship status.

You will need to upload the following documents:

  • Identification document of previous citizenship such as both sides of your old NRIC, Student’s pass, Dependent’s Pass
  • Identification document of current citizenship such as both sides of your new NRIC, Student’s Pass, Dependent’s Pass
If you need to submit a JAE application on behalf of someone

You can request to submit an application on behalf of the applicant when they are unable to register during the JAE registration period.

You must be a parent, guardian or an adult aged 21 and above (i.e. a proxy) to do this submission. You will need to upload the following documents:

  • Both sides of your NRIC or passport.
  • Both sides of applicant’s NRIC, birth certificate or passport
  • Applicant's O-Level certificates or result slips
  • Applicant's CCA certificates, if available

If you are not the applicant's parent or guardian, you must provide these additional information:

  • Both sides of applicant's parents/guardians' NRIC or passport
  • A completed proxy form (123kb)
If you need assistance relating to O-Level subjects or grades

You can request for assistance relating to O-Level subjects or grades.

You will need to upload the following documents:

  • Both sides of your NRIC
  • O-Level certificate

Note:

Guidelines for submission:

  • Supporting documents can be in the form of jpegs or pdfs.
  • Compress the documents into a zip file before uploading. The maximum file size should not be more than 7mb.
  • Late or incomplete submission will not be processed.

2. What you need to do after the request submission

Ensure that you remain contactable via phone and email. Our team will get in touch to follow up on your request by the next day during the JAE registration period.

If you are uncontactable, we will not be able to process your request and will not consider any replies after the JAE registration period ends at 4pm on Monday, 17 Jan 2022.
